For Providers
It is widely known that multiple medical, demographic, social and cultural factors influence diabetes care, adherence to treatment and outcomes, yet very few programs address all these aspects in routine clinical practice.
Joslin's Latino Diabetes Initiative provides culturally appropriate diabetes care and education, research, community outreach and provider education to improve health outcomes for Latinos living with diabetes or those who are at risk for developing the disease.
Our strategy is to improve knowledge and expertise among community-based health professionals and paraprofessionals through training that is accessible and applicable to the needs of all levels of health providers serving Latinos. As part of this effort, we provide information about diabetes prevention, education and treatment for Latino populations. In addition to our education programs, we also work collaboratively with referring physicians to co-manage patients with diabetes. Working together we can help our Latino patients reach their therapeutic goals and live a better life with diabetes.
